New Mexico Medical Cannabis Cards

Partnering with Grady's Cannabis in Aztec NM, Mountain High Medical offers New Mexico medical marijuana cards and medical cannabis card recommendations for the qualifying conditions listed below to patients 18 years and older with a valid NM Driver's License or NM ID. 


Renewals $80 (Must have current NM Med Card)


New Cards $100 (With existing diagnosis)


New Cards $120 (Requiring a diagnosis)

Patients under 21 please see below for details regarding special dispensary entrance instructions.


  Please note that we only offer appointments one Wednesday each month from 10 am to 3 pm and can only be booked through this website.  


A Non-Refundable Deposit is required to book appointment, please only pay the deposit required to book the appointment as it's NON-REFUNDABLE and any additional money paid will also be NON-REFUNDABLE.

2) Register / Log-in to the New Mexico online Patient Portal


Please remember to bring the following items / documentation 


1) Your valid NM Driver's License / ID 


2) Your medical diagnosis from your primary care provider (PCP), if you are applying for a New Mexico medical card with existing diagnosis.


3) Smart Phone to check email & uploads

(Must have a unique email address)

MUST HAVE A SMART PHONE & EMAIL ADDRESS


4) If you are a minor under the age of 18, please bring legal guardian & birth certificate.  


If you are a minor between 18 - 21 you must be escorted from Grady's Cannabis entrance to the back appointment area, please let the front door staff member know that you are a minor coming in for a scheduled appointment.


5) Your current New Mexico medical card if applying for a renewal.

  2. Amyotrophic Lateral Sclerosis (ALS)
  3. Anxiety Disorder
  4. Autism Spectrum Disorder
  5. Cancer
  6. Crohn's Disease
  7. Damage to the Nervous Tissue of the Spinal Cord (with objective neurolog
