Medical Cannabis cards

Partnering with Grady's Cannabis in Aztec NM, Mountain High Medical offers New Mexico Medical marijuana cards and medical cannabis card recommendations for the qualifying conditions listed below to patients 18 years and older with a valid NM Drivers License or NM ID. 

Renewals $80 (Must have current NM Med Card)

New Cards $100 (With existing diagnosis)

New Cards $120 (Requiring a diagnosis)

** Veterans receive a discount **

Patients must be 21 years old to enter Grady's Dispensary. 

If a patient is under 21 and has a legal guardian, please call ahead to make  alternative appointment arrangements.

 Please note that we only offer appointments one Wednesday each month from 10 am to 3 pm and can only be booked through this website.  

A Non-Refundable Deposit is Required To Book Appointment

1) Your valid NM Drivers License / ID 

2) Your medical diagnosis from your primary care provider (PCP), if you have one

3) Smart Phone to check email & uploads

MUST HAVE A SMART PHONE & EMAIL ADDRESS

4) If a minor please bring legal guardian & birth certificate
