 Please note that we only offer appointments one Wednesday each month from 10 am to 3 pm and can only be booked through this website.  As a result of limited appointments, a Non-Refundable Deposit is Required To Book Appointment

Please remember to bring the following items / documentation 

1) Your valid NM Drivers License / ID 

2) Your medical diagnosis from your primary care provider (PCP) 

3) Smart Phone to check email & uploads

MUST HAVE A SMARTPHONE & EMAIL ADDRESS

4) If a minor please bring legal guardian & birth certificate
